Admissions Clinician

(Full-Time) Reporting to the Program Director the Admissions provides screening and assessments, makes provisional diagnoses according to DSM-5/ICD 10 criteria, provides level of care recommendations under ASAM and advocates for patients. Admission Clinician conducts mobile assessments, outreach activities, and participates in designated meetings, conferences and speaking engagements under the direction of the Program Director.  Admission Clinician will be responsible for working with the clinical team to obtain initial and ongoing authorizations for treatment services.

RESPONSIBILITIES:

  • Performs clinical assessments for admissions to Phoenix House programs; performs assessments and evaluations in keeping with Phoenix House policies and procedures.
  • This includes, but is not limited to interviewing prospective patients and relatives to obtain necessary and pertinent personal and financial data to determine eligibility and appropriateness of treatment and admission.
  • Completes progress notes, and documents provisional diagnosis and medical necessity for treatment in client's Electronic Health Record and County systems, as applicable.
  • Performs crisis intervention sessions for families who are seeking treatment.
  • Completes the admissions process (which may include transportation) for assigned intakes with approval by their authorized representatives.
  • Creates the patient's Electronic Health Record, as part of the admission process and responsible for data management related to assigned county , in-network insurance provider, and/or or private pay patient individual charts,
  • Maintains and provides a list of referrals for patients who do not meet the admissions criteria.
  • Participates in both daily admission meetings and bi-weekly individual supervision meetings with Program Director.
  • Communicate and coordinate with Program Director regarding prospective intakes.
  • Actively establishes and maintains contacts and communications with various organizations and agencies to increase referrals.
  • Maintain all required reporting functions and assure 100% completion of all admission paperwork as required by the agency, county, and other regularly bodies

REQUIREMENTS:

EDUCATION/CREDENTIALS/EXPERIENCE:

  • Must possess an AMFT/ACSW/LPCC and have an active BBS registered intern number working toward licensure.
  • LMFT/LCSW, preferred
  • Familiarity with LPHA, ASAM and DMH documentation and billing, highly preferred.
  • Must maintain a valid CA Drivers' License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy

  • Demonstrated strong verbal and written communication skills
  • Proficiency in MS Office products, strong typing skills, and familiarity with EMR, including effective data entry skills
  • Ability to adhere to the professional ethics and conduct of standards as established by professional licensure, the policies of Phoenix House, and legal, funding and regulatory requirements
  • Bilingual (English/Spanish) highly preferred.
