Reporting to the Wraparound Program Director, the Wrap Coordinator is responsible for the the timely, effective performance of the Wraparound team to provide appropriate case management and advocacy services for assigned clients in accordance with their treatment plan and needs. Effectively partnering and coordinating with community resources to ensure clients access to needed services. Case management, counseling and advocacy services to assigned clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Performs clinical supervision to the Child and Family Specialist, and the Parent Partner staff to ensure they are implementing the principles of Wraparound, Core Practice Model, performing effective individual mental health rehabilitation, and case management services.
- Assist the Program Director in identifying training needs of staff; works with others to develop and implement orientation and training program for Wraparound staff; provides direct orientation and training as needed.
- Reviews practices, policies and procedures of Wraparound program to ensure compliance with agency expectations. Demonstrates knowledge of DMH EPSDT billing protocol as implemented within the Wraparound Program.
- Assist the Program Director in demonstrating effective management of MHS required forms, chart reviews and chart updates in a timely manner and in accordance with agency and county protocols.
- Assist the Program Director with ensuring each billable staff maintains appropriate caseload to reach the budgeted daily expectation of billable hours and coordinating required documentation with Wraparound staff in timely manner.
- Participates in Quality Improvement/Utilization Review activities and performs related duties as assigned.
- Collaborates and coordinates with other community professionals, servicing agencies, clients, and families.
- Serves as a secondary and back-up support for the Program Director contact for children, families, service providers and the community, acts as a liaison between client, agency and community resources.
- Maintains a personal caseload, providing the full range of case management and advocacy services performs any or all the duties of a Facilitator and Therapist, as needed.
- Provides crisis-intervention type of services as needed either in response to requests from another staff member(s) or as a result of personal observation.
- The incumbent in this provides clinical supervision to the Child Family Specialist and Parent Partner Wraparound employees.
- The incumbent in this position provides supervision to clinical trainees.
- Confirms quality service delivery through completion and submission of documentation (progress notes, service plans, assessments, treatment plans, notes to chart). Reviews and signs all contract required clinical documentation.
- Ensures each billable staff maintains appropriate caseload to reach the weekly expectation of patient contact and budgeted hours.
- Develops positive workplace relationships in accordance with Phoenix House Policy and Procedures with staff and peers by offering appropriate support within a nurturing environment while maintaining a high level of professionalism.
- Attends all required staff training sessions in accordance with Phoenix House and state licensing requirements, completes all training on time. Completes 40 hours of in-service training annually.
- Adheres to the professional ethics and conduct of standards as established by professional licensure, the policies of Phoenix House, and legal, funding and regulatory requirements.

Applicants must meet one of the following qualifications:
- Master's Degree in Social Work, Marriage and Family Therapy or related field
- AMFT or ACSW with current California Board of Behavioral Sciences (BBS) Registration required.
- LMFT or LCSW with current California Board of Behavioral Sciences (BBS) Registration required.
- Must maintain licensure as a condition of employment.
- Evidenced-Based Trained/Certified in MAP, TF-CBT, SS Preferred
- Must maintain a valid California Drivers' License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy

- Bilingual in Spanish preferred
